چکیده

THE FAILURE of a cardiac impulse to traverse the entire atrioventricular (AV) conducting system is called concealed conduction. Electrocardiographically, the effects of concealed conduction are reflected in the conduction and/or formation of subsequent beats.1 The concept of concealed conduction can be applied to explain (1) post-extrasystolic prolongation of the P-R interval, (2) the irregularity of the ventricular response during atrial fibrillation, (3) the slower ventricular response during atrial fibrillation as compared to atrial flutter in the same patient, and (4) alternation of the P-R interval, etc. The recording of His bundle electrograms has permitted us to localize more precisely the area within the A-V conduction system where the impulse is blocked and also the area where conduction of subsequent beats is prolonged.2 By means of His bundle electrogram recordings, the P-R interval can be divided into the A-H (atrium-to-His bundle) interval which is a measure of A-V nodal conduction time, and the H-V (His bundle-toventricle) interval which is a measure of conduction along the His-Purkinje system.

Supernormal conduction in the accessory pathway of patients with overt or concealed ventricular pre-excitation.

It was recently shown that supernormal conduction in the diseased His-Purkinje system is more common than previously thought, and is always associated with prolongation of refractoriness. To assess whether supernormal conduction could also occur in the accessory pathway of patients with ventricular pre-excitation, 21 patients with manifestly prolonged refractoriness in the accessory pathway wer...

Demonstration of entrance block into the atrioventricular node of man.

was demonstrated in five patients by using programmed atrial stimulation and His bundle recordings. Entrance block into the A-V node occurred in a narrow range (28-33%) of the basic cycle length and concealed conduction occurred at coupling intervals longer than this range. It appears that entrance block of these early APBs is due to a "functional block" between the atrium and the A-V node. The...

Modulation of extracellular atrioventricular node field potential pattern and ventricular rhythm by morphine in experimental atrial fibrillation in isolated rabbit heart

Introduction: Endorphins are produced by cardiomyocytes, and exert different effects on the heart. The aim of the present study is to assess morphine effects on extracellular atrioventricular (AV) node field potential pattern and ventricular rhythm of isolated rabbit heart during experimental atrial fibrillation (AF). Methods: Effects of different concentrations of morphine (10, 20, 50 and 1...

Frequency-dependent electrophysiological properties of concealed slow pathway of isolated rabbit atrioventricular node preparation after fast pathway ablation in a functional model

Introduction: Intranodal pathways of atrioventricular (AV) node play a vital role in the delay of conduction time in response to various atrial inputs. The present study was aimed to determine the frequency-dependent electrophysiological properties of concealed slow pathway according to a functional model of isolated rabbit atrioventricular node preparation after fast pathway ablation. Meth...

Limitation of tachycardia zone resulting from longitudinal dissociation of the atrioventricular node in concealed pre-excitation.

Two cases with a concealed left-sided accessory atrioventricular bypass tract are described. In both, functional longitudinal dissociation of the atrioventricular node narrowed the range of atrial premature beat coupling intervals which could initiate re-entry using the accessory pathway. In case 1 early premature atrial beats were followed by an atrioventricular nodal re-entrant echo. The atri...
